Description

NUCLEAR CARDIOLOGY FOR THE CBNC EXAMINATION AND THE READING ROOM

A reversible inferior defect may represent RCA ischemia, diaphragmatic attenuation, subdiaphragmatic activity, patient motion, attenuation-correction misregistration, or a processing artifact. The perfusion slices alone may not provide the answer. Correct interpretation may depend on the raw projection data, gated wall motion and thickening, CT alignment, stress response, tracer kinetics, and clinical history.

This is also how the CBNC examination approaches nuclear cardiology. Successful preparation requires more than memorizing criteria and protocols. It requires understanding the physiology, physics, pharmacology, instrumentation, artifacts, and clinical reasoning that determine what an image actually means.

This book was written for cardiology fellows, nuclear cardiology trainees, and practicing physicians who want to prepare efficiently for the CBNC examination while developing a practical approach to nuclear cardiology interpretation.

CLINICALLY ORIENTED, BUT EXAM FOCUSED. The emphasis throughout the book is on problems encountered both on the examination and at the workstation.

A fixed perfusion defect with preserved wall motion and thickening requires a different interpretation from a fixed defect with regional dysfunction. A strongly positive PYP scan cannot establish a non-biopsy diagnosis of ATTR cardiac amyloidosis when evaluation identifies a monoclonal protein. Diffuse myocardial FDG uptake may reflect inadequate physiologic suppression rather than active cardiac sarcoidosis. An unusually high gated SPECT ejection fraction may reflect underestimation of end-systolic volume in a small ventricle rather than truly supranormal systolic function. Abnormal PET myocardial blood flow requires assessment of technical quality and physiologic plausibility before the numerical result is accepted.

These distinctions are difficult to learn from isolated facts because they depend on understanding why the finding occurs.

The same principle is applied to technical material. Counting statistics, radioactive decay, attenuation, spatial resolution, scatter, coincidence detection, tracer kinetics, and PET quantification are explained in terms of their consequences for image acquisition and interpretation.

DESIGNED TO HELP YOU PASS THE EXAMINATION The CBNC examination frequently presents information that can be interpreted correctly only when several concepts are connected. A perfusion finding may require gated data. A PET flow value may require knowledge of tracer kinetics or technical limitations. An amyloid scan may require integration with laboratory testing. A stress complication may require understanding the pharmacology of both the stress agent and its potential reversal.

For this reason, the book emphasizes a consistent approach: identify the finding, determine the mechanism, examine the supporting data, recognize the important alternative explanation, and select the most defensible answer.

This reduces dependence on memorizing every possible presentation and provides a practical method for approaching unfamiliar board questions.

The goal is straightforward: prepare thoroughly for the CBNC examination while learning nuclear cardiology in a way that remains useful in clinical practice.
